2 part kitchen floor

Greetings oh wise ones.
I have a kitchen floor that is half concrete and half wood. The covering at the moment if vinyl floor tiles over two thirds and carpet on rest. My wife would like tiles over all of it. The problem I see is that the transition between wood and concrete is not smooth - the vinyl tiles on this have cracked along the line between the two.
How to I cope with this?
Do I use the transition line between the two as a seam?
The level is not very noticeable, but I don't know how to measure it to give you a better idea of my dilemma.
Any help gratefully received.

Put a level between the two transtion to see what you have to cope with and bridge that part with one tile if you can, have a bit of a dry run before to see how the tiles are going to work out. You could always lay exterior grade plywoood on the wood floor to bring it up to level, but nail it down well about, 6" apart all over. ;)
