concrete foundation for summer house

Help. I need to build a concrete foundation for my summer house as I have just moved and I took it with me.

I was thinking of just using 1 ton bags of ballast and cement. if so how do I work out how much i need and what is the mix.

Also is it best laying the concrete level to the grass or above by 2 or 3 inches. if its above i will need timber to support the conctrete around the edges unlike just digging the hole and pouring in the concrete and smothing it of.
