A new build converted the roof, we had a significant water leak, does the attached look ok?

And also water was pouring down from fireplace, what could be the cause?
 
The outlet where water comes out, all over the metal flashing. Where does it (the water) go after that?
 
Please see attached photos, the water travels down the metal flashing to a scrupper drain (?) the concern is that it was heavy rain and all that water comes down through our roof caused leak. Solution provided is to have another two drainage holes on our roof. Should the outlet be connected to another pipe so that the water is not directly falling on our roof? And is the metal flashing looking ok? Thanks!

Hard to tell what goes where in the photo, the flashing obviously obscures what's underneath so impossible to say where the fault may be.
But yes if overwhelmed by a deluge then the water could back up in the channel and breach the structure.
I wouldn't be happy having that downspout there, I'd prefer a pipe taking the water out of the way of everything. Down and away.

Another issue could be that there is far too much surface (catchment area) on the roof above coming through that single downpipe.
 
Get a proper roofer in to check it all out.....seek word of mouth etc.
