What is the correct way to join a solid wood floor (13mm thick) to a carpet via a door bar? Should the existing metal gripper be removed and replaced by an appropriate wooden bar or should the metal gripper stay in place and the new flat bar fitted over the top?

My guess is that the gripper should be removed but that means there would be no gripper holding the carpet across the door way.

Remove the metal gripper and fit a wood Square edge profile, refit the carpet against the front of the square edge profile by fitting a new piece of carpet gripper accorss the face of the nosing with a 10mm - 15mm gap to allow the carpet to be tucked down against the profile.

Don't be tempted to fit a T shaped profile, it will snap on the carpet side.
