Our downstairs toilet is positioned 6ft away from garden manhole. Does it need to be vented / AAV? Please see photos. Thanks.

Only if you're having issues with it!
Hi, no issues and smells to be honest. However, we are looking to install an adjacent utility sink, I am not sure if a new boss pipe will be needed, currently everything is buried under screed. Would the sink need vent and how would it connect to toilet waste pipe - is it easier just to create separate sink waste to manhole? Thanks for your time responding.

Well that changes things from OP.
Obvs you'll need a way to connect sink and that needs an idea of layout and positions which your photos don't show.

The new sink would be positioned just behind the door. Thanks.

Need to see a lot more to be able to advise properly please. How is the WC connected to the drain? If old type pan with outlet joint made with tarred rope and mortar then life becomes more interesting. If Pan has been replaced in the past, and Plastic Pan Connector fitted, that can usually be fairly easily swapped out for a Connector with a Boss, which will allow connection of a basin waste.

If you can answer that , then we are better placed to advise on your next steps.
