I've a traditional porcelain toilet and the flush often needs a few tries to work. I've taken the top off but can see no obvious difference what happens the times it works or doesn't. Nothing is slipping and the action feels normal, it just doesn't 'grip' and trigger the flush.

What should I look into?

If it is a lever operated siphon flush ,the diapraghm is probably split and needs replacing. Can you post a pic of the inside of the cistern ?

Time to replace flush unit, change to a Flapper valve, while you are about it change the filler valve, Fluidmaster Brass shank fill valve.
