Extension build issues

Hi,

We currently have a lean-to extension across 1/2 the back of our house, and are now extending this across the full width.
The previous foreman onsite identified an issue with the new walls, which were constructed before he arrived.
The inner skin is 60mm out of line with the existing internal wall. It appears this is because they built the external face of the wall inline with the existing, and didn’t realise the existing wall is faced with brick slips. The existing wall is brick slips, breeze block, insulation, breeze block whilst the new wall is brickwork, insulation, breeze block.
This is an issue as the roof sits on the inside skin.
Is this an understandable mistake, or does it bring into question the competence of the builder?

The second concern I have is the way the new walls are constructed. The walls are brick at the bottom, then a creasing tile, then rendered block work. The creasing tiles of the existing walls extend through the full depth of the wall, so both inner and outer blocks sit on the tile and it spans the void.
For the new walls the creasing tile sits on the outer brick, a tile sits on top of the creasing tile but offset back so it extends over part of the void and the block work sits on top of this top tile. This means the blockwork is only partially supported. Is this normal?

It’s not an understandable mistake, sounds like a total c0ck up.
Is the insulation requirement in the cavity being done to meet current regs? Might mean the new cavity needs to be wider than the old one.
I don’t really get the creasing tile across cavity thing. Never seen it before.
