Floorboards Nailed to Concrete?

Problem: Wish to sand and finish c.60 yr-old floorboards but find they have been cut-nailed to a concrete 'screed'. This has been treated with some sort of black bitumen and doesn't seem damp but the nails have rusted and rotted the boards around the heads of a lot of them. There are also up to 10mm gaps between some of the boards. The holes and gaps have previously been filled with white filler; the room was 'carpeted'. Problems of colour matching/blending new boards/filler accepted -
I can think of various solutions: get them all up - replace the really bad ones - re-lay with adhesive (or re-nail) and fill any remaining cracks & holes; Leave all but really bad ones alone and do major crack and hole filling; lay a new floor on top; lay carpet again & move; but was curious as to whether anyone had encountered this type of flooring and their experiences with it.

Ok, this is my idea, if you have time, take all the boards up, screw 1/2 inch (12mm) battens to the floor (30cm centres? not sure about this). put insulation between battens, if sub floor dry enough (<12%) Nail floor to battens using hidden nailing. Should look top!
