How do I fill this large gap between new external door threshold and floor?

Hello, hoping someone can help! We've just had a new garden door fitted (where there wasn't one before). There is now a big gap (2"+) between the internal wall of the house and the door. How do I bridge this gap so that it is secure (doesn't move around / squeak every time you tread on it) and looks OK when connected to the internal flooring (the black tiles)?

There is also a small height difference (approx 2cm) between the tiles and internal wall / threshold that can't be seen very well on this pic.

Any advice would be greatly appreciated!

Is that the cavity in bricks? All my doors had the cavity which was covered in felt to bridge and floor screed over.
I removed screed then felt.
Thin polystyrene against outside wall and the bottom.
Plastic DPC. That was a rubble stack.
Poured concrete in to fill
