How to put in 6" lintel

The struct engineer has calculated a 6" is required, BC only want to come and see the lintel when it's inserted and say they can't tell me how to do it.

I'm happy putting in the acros, strongboys and putting in the lintel with a 20 by 20 padstone, but a few questions:

1. What mix of concrete should I use for the padstone and does it need gravel?

2. I've heard talk of using slate pieces. Is that necessary with a padstone for a lintel if I get the top of the padstone flat and level?

3. Should I use a bed of mortar on top of the lintel when I insert it?

1. You don't mix anything, just go and buy a suitably sized padstone from any builders merchant.
2 - If the stones on both sides are 100% perfectly flat, and the lintel is also 100% perfectly aligned then no packing required. In the real world, some packing material may be needed, slate being a common choice.
