installing wall ties to new cavity wall. What height

I plan on building my walls above DPC from tomorrow which consist of 2 skins of blockwork with 90mm cavity (40mm celotex).

Below DPC is 2.5 blocks high which is filled with concrete to 1 block below DPC (ground level).

Question is at what height should i install the first wall ties. Should this be the first course (on the DPC) or second course etc? How far apart should they then be spaced horizontally and vertically.

Also, one quick last question, should the insulation start from above the DPC, or should it extend down to the concrete?
