Laying new floor

Hi all

Im going to lay a new concrete floor but have been told i should put some sort of insulation down before i put the concrete in. is there any where that i can get an idiots guide on the depth of each level.
i.e. how deep should the 1st concrete level then the DPC then the insulation then what do i cover it with and how deep.
