Mixer bar shower leak help!

I have a mixer bar shower that leaks when turned on at the outlet of the mixer bar, I have tried new washers and ptfe tape, aswel as liquid ptfe, but every time I turn it on water starts leaking from the joint and it takes a good few seconds for the water to start coming from the shower head, any ideas please?

There's usually only 3 reasons for that,

1) the sealing washer between the riser and the outlet connector is leaking due to it either being damaged or the new ones are too thin
2) the nut on the riser isn't tightening down properly
2) the outlet fitting in the bar itself has become loose from the bar and it's leaking past its seal, sometime happens.

You've tried 1, so have a look @ 2 or if that's ok then 3 and see if the outlet connector is loose in the bar.

The reason it's taking a good few secs for the water to appear @ the shower head is over time the riser column will be emptying of water due to that leak and the shower then has to make that back up again before the water gets to the shower head, hence the delay
 
Thanks for the reply, in regards to not tightening properly, before I put the shower arm in its holder with the grub screw I'm able to rotate the shower bar very easily even though the nut is tight, could this be one of the reason the water is leaking from the fitting.
