We are having a single storey extension. We want a toilet in it, to connect to the main drain is impossible as it's encased in concrete due to being in a mining village (we think that's why it was encased). It might be possible to connect to the drain in our house but that would mean having an internal manhole, are these any good & can you disguise them. We are putting wooden flooring down & would be able to cut out the size & lay it in the space & hope it just looks like a saw cut rather than having a metal edging, but we don't know if this is allowed. Our only other option is to have a macerator toilet, but the reviews we've seen are horrendous. Any suggestions would be great. Thanks

Depends on age of property, drains were encased in concrete as a matter of course at one point in time.

Internal chambers are not ideal, but if needs must then its probably a necessary evil. You must have an internal grade cover fitted, which is double sealed, and screwed down shut. These are designed to allow the floorcovering to be incorporated into the lid, and provided the installer and/or floorlayer is competent, they are pretty inconspicuous, with just a thin metal line where cover meets frame visible.
