Does a aav have to be fitted at the end of a soil stack run. Or could it be positioned as the 4 inch pipe ruins horizontally, via a boss with a 2 inch pipe coming off this. I won't to do this to keep any boxing in to a minimum.

Depends on what the connections on the foul run are and why you need an AAV rather then a vent.
If it's definitely an AAV then it must be fitted at least 300mm above the highest overspill level of any appliance connected to it i.e. 300mm higher than the lip of a basin etc.
