Repair Inside Patch after outside crack repair

Hello Expert DIY :)

A damp issue was repaired which was caused by an external crack which one small area of the internal walls to bubble and paint flaking off.

I have scrapped the loose paint and apply filler and repainted.

but now a week later, as you can see the paint is powdering, flaking off again.

Am I missing something?

Thanks in advance to anyone for your expertise.

That's looks wet. Needs further investigation to determine where water is getting on. Could be roof and running down cavity?
 
it use to be damp but now it is dry when hand on the wall. what we see as damp is more the colour difference on the paint. (as not done a good job one could say)
I am more concern on the paint peeling powdering - little bubble and when touching the surface, it is like powder.

does that means the wall is not dry enough and I have painted too quick??

Now, 3 weeks after, this is how the wall look like... kind of how it was before... no clue what to do ...
Weather has been quite good - so what is causing this?
