Tiling onto concrete

I'm tiling onto concrete floor downstairs toilet. Room is small (using large tiles 45x45cm) and so 8 tiles cover the whole floor. Where I have taken up the carpet and pulled up the carpet gripper there are some holes in the concrete (mainly around the edge but some around the original toilet) where the nails have come up. However when I place the large tiles on the floor they lie flat. Do I need to fill in the holes or will putting adhesive just do that anyway? Also, what primer should I use?
