Tiling over old Victorian front path

Our front path has seen better days (see the picture) and, having done a bit of internal tiling, I thought I'd take on the job myself.

I have tried taking up a few tiles and while some came up easily, others only came up with a chunk of the underlying concrete. Surprisingly, I can't seem to find any advice or answers on the net.

So my question is, can I just tile over the existing tiles or do I need to rip up all the tiles and, as the concrete slab underneath will probably be knackered, redo that as well?

A number of things:

Do not tile over the existing tiles.
Are all the remaining tiles well stuck down? Apparently not.
How thick is the concrete sub-base?
Given the cracks in the sub-base and the bumpy tile surface it would be best to perhaps lift all the tiles.
It might be best practice to root out the base, and begin again from scratch?
You will need a fresh edging.
The stone step is cracked and has what looks like a concrete infill.
Is the party path wall in a sound condition?
