transit van stall when stop or slow down to change gear

My van is a bit old, with 189k miles on the odometer. It has had issues with the gearbox and has been burning oil for the last two years at least since I bought it.
Recently, it has started to stall when I stop to wait at junctions, slow down to a crawl, or at roundabouts. It drives perfect fine on the motorway, but the problem returns when driving in the city after one or two days. I would like to know if there are any possible faulty parts I should check or replace before taking it to a garage. Thanks!

It also makes strange noise after engine turned off for couple of seconds
 
The service was done last October or November, but only the oil was changed. For some reason, the mechanic couldn't replace the oil filter because it couldn't be unscrewed.
The engine starts perfectly fine. There used to be smoke with a bad smell when starting, but recently the smoke seems to have disappeared, and the smell is less noticeable as well.

A wild guess........the EGR valve is stuck open.
Are there any warning lamps illuminated, and has the van had a diagnostic check for DTC's? (Diagnostic Trouble Codes)?

I also think the symptom might be caused by the EGR valve as perfect driving on the motorway. However, I am a bit worried that it could be cylinder scuffing, since the van has been burning oil for a long time, probably long before I bought it. I haven't taken the van to a garage to check yet
 
Check the throttle position sensor, if it's an electrical type.
The symptom look very similar to that due to faulty TPS. I will try to reset it by disconnect the battery if it get better
 
Any more info on what the van actually is, engine wise and age ?

The TPS sender on the pedal of my wife's Corsa some years ago caused problems. The resistance tracks had worn through at/near the minimum throttle position, so at low revs the ECU got confused. No probs at higher throttle settings (which obviously see less use).
