Uneven concrete floor

I am soon to be laying some solid oak flooring, but the concrete it's going to be laid on is not very flat. Can I screw 12mm ply to the concrete to take out most of the unevenness, and if so, what else should I do to make a 'proper job' of it?

Sorry, I forgot to mention that the floor level can't be raised any more than the thickness of the ply and the oak flooring combined!
