Sorry about the quality of the picture, it was from the scanned paper.
I can view the on line version but its in an Adobe Flash viewer. I spent ages trying to find a way to copy it and ended up at an Adobe forum where one of their software engineers effectively says that the information provider DONT want people to download it.
