I take it you are referring to the 117mm linears.One firm I worked at kept the 110V and 230V 500W halogen lamps next to each other in the stores. I think everyone who worked there at some point grabbed the wrong voltage lamp and put it in a 230V fitting.

Oh yes those do make a pop don't they. I had a stock of both and a colleague had selected the wrong ones when we decided to replace all 4 in a halls working lights. We wouldn't usually do that but we knew one or two had blown and power was off for a refurb so we took advantage of the scaff towers. I was working off a step ladder on stage when power was restored and thought the roof was collapsing or something else major. Annoyingly 110V linears were significantly more expensive than 230V by a factor of several.
