Please see the following. in particular page 27 which shows the wiring diagram. You need to verify the wire exists from programmer HW OFF to C/S (cyilinder Stat) and the wire from C/S to the grey wire of valve. It's the omission of these that prevents the valve moving from mid point onwards.
The actuator head contains two micro switches making 4 pairs of contacts which are just plain switches. Any pair may fail by either not making contact or by not breaking contact. If a pair of contacts don't break open it obviously means a pair don't close
This can either cut the power to drive the motor forward or prevent power out from the heads 'orange' wire to light boiler.
