i have had several of these heat exchangers leak like this

anything that uses that big square h/e. quite a few of the greenstars models used them befroe they started using that pain in the arse tubular h/e.
usually the obvious signs are an awful lot of topping up and the biggest plume you have ever seen from a condenser.
simple to change though.
