What about extracting through the ceiling, with a fan in the loft?

Free of the constraints of an in-wall one you could put a large powerful one up there.
I hadn't thought about that. Having just had a look at some, I'm annoyed that you can get ones with lights in the centre so could have got that installed instead of the downlight.

As ban eluded to, don't get one of the ceiling mounted fans that have a built in light because they are not very powerful. What you need is a loft fan which sucks one side and blows the other. With these all you need is a ceiling inlet and an outlet in the roof. They are quieter too.
