I am looking to put glass separation wall.
Where can i get clear grid or reeded glass like below with wood frame?

That's certainly not a mainstream glass, can't see I've seen it in over 20 years in the glass industry. In fact reeded glass has only just been reintroduced after 20 years of not being produced , but that's not in squares like your example. Possibly need to have it custom made

Sorry, it's not glass, but could you get the effect you want with acrylic?
A similar pattern is common on the prismatic diffusers on light fittings.
