I would have liked to have seen better bonding of the masonry, either side of that lintel. There appears to be a long run of straight nick perp's either side of the lintel.

Sorry it's taken a while. I was working away when I originally put the post out and then forgot as we are having quite a bit of renovation works going on!
The main one is we're the fire must have been. The boiler relief valve is what it is and BT busting the brick out, I can buy a brick buster to cover that (his method was smearing it full of clear silicone and "sticking it back on" )
There is other bits here and their where the old waste is being filled and new redrilled. Just trying to make it less noticeable.

1. Replace the brick or decide not to care about it, depends how visible
2. Wash with brick acid after protecting the pipe. Will probably look fine after as the pointing's pretty neat other than the surrounding mess
3. Cut out all part bricks and replace with 8 matching bricks...
Botching about with filler won't help or may make it look worse.

Agreed, but I know that he will have mixed in the correct ratio of powder/PVA/water.
