Does anyone know of, or better still, have a copy to borrow, the format of a HOT WORKS PERMIT please?

We are having some work done next week and our insurers now insist on this item, but have given us no idea of what it looks like,or where to get one, plus the engineers have now asked us for one as well!

As usual, I'm the guy stuck trying to get everything covered!

What is it you're having done ? Is it a chemical plant ? The permit is for to prevent a major accident.
We use "Hot Work Permit" in the Oil refinery for work with flame cutting, electric grinder, blow lamp, welding etc.
I don't know where you can get one from, you could try the local building control department or see here for more information.

No, it's some welding and iron work going on at the (external) rear of the building and we have, apparently, to issue the firm with one of these Hot Works Permits.

Also, from what I've found out so far, it's issued a day at a time? Is this right, does anyone know please?

usually have to be signed on in the mprning and off at night on all the sites ive worked on.
Its basiclly to cover you if there is a fire
IE all flammable material should be removed and fire extingishers provided by the contractor

And before you tell me i know in the real world this is usually impossible

Our Hot Work Permit is 24 hrs and also the fireman does all the gas tests & safety checks in a operational area of 13ft radius. I don't know if you have to involves your local fireman but they maybe able to advise you. Don't ring them on 999 though

I may have one at home if you can wait until Monday. Alternatively you can make up your own. The idea is that the form assigns responsibility for safety to a 'responsible person' who has to be a named individual rather than a company. The form is normally issued by or on behalf of the owner or operator of the site and is signed by the 'Authorised Person'. The permit can be of any duration but is usually for a working day - the responsible person is, err responsible for the entire duration of the permit so don't accept one for a 24 hour period if you want to go home at the end of your shift! The form should have a description of the site, the 'working area' and the fairly precise nature of the work to be carried out. It can include sketch plans if that is an easier way to describe the area.

A copy of the form should be retained by both the responsible and the authorised person for the duration of the project and usually added to the health and safety file. The form is intended to be a formal record of responsibility in the event of an accident and HSE involvement. I would guess that the project is not notifiable under CDM regs so you don't need to write an H&S plan but a contractors method statement and risk assesment for the work would be good if it is complicated or of an unusual nature.

Good luck.

Yes please, I'll certainly see about knocking one up in the meantime, the work starts on Monday, but the hot works isn't due to start until Wednesday

GOOD point!

Thanks for all the above, got the contractors method statement and risk assessment yesterday. And authorisation to "cobble a form together and sign it on behalf of the firm!" Gee, ain't I lucky!

Mr. Dragon - I've found a permit to work but it is a 'Petroleum' rather than a 'Hot Works' and is an MoD document so is much more complicated than it needs to be. I could email it but have no idea how to obtain your email address from here.

Thanks for the offer, but I don't need "more complicated that it needs to be", simple is the key!

That's a point.... Don't want to put my email addy in any forum, ends up being harvested too much of the time. Thanks for looking regardless!

MUCH appreciated.

The problem is, there are many difference type "Hot Work Permit" form for certain job.

See this and there is a freephone number from Safeguard Systems who maybe able to help you.

Freephone 0800622322

I have one in front of me but no good for what you want.

I've checked at home and only have more petroleum, confined space, medical gas and airfield permits to work, the format for all of them is similar and could be re-written as a hot works. As you say, complicated isn't what you need but you're welcome to one of these if you want. You could remove 90% of the irrelevant stuff like atmosphere testing and nitrogen purging. Saying all that, I think that in your position I would probably start from scratch.

I understand your reticence for putting an email address in your profile, if you do need a copy of a PTW, I'll briefly put up an address - It's one I use for this sort of thing and gets bombarded with spam but I just open it every so often, pick out thee ones I'm expecting and delete the rest.

Regards, PTH.

Hi PTH

Thanks for the offer, I've been given a nice simple one that suits the welding/grinding work to be done down to the ground. Just had to type it up and lay it out for our own use, see how it goes this week now!

Thanks to everyone for their help! MUCH appreciated!
