X4 is volts free.
"Volt free" is a very confusing description because it does not mean that there is no volts between the terminals. All it means is that the voltage between the terminals is not 230V AC.

Connector X2 takes a mains thermostat, but a "volt free" one can also be used.

Connector X4 works at 24V DC, so you must use either a "volt free" stat on terminals 6-7 or an Opentherm stat on terminals 11-12.

As for the question whether the Room unit of an opentherm stat uses batteries, it might and it might not; there is no hard and fast rule. The OT spec says:

It is the intention that OpenTherm provides suitable power from the boiler unit to the room unit such that no additional power connection or use of batteries is required for the room unit.
3.3.1 Options
The options for supplying power to an OpenTherm room unit are :
i) line (OpenTherm-supplied) power
ii) local supply (mains, batteries or other independent power source)

Any OpenTherm room unit is permitted to use line-power (provided that its requirements are met by the available current and voltage), or are free to exercise option (ii) above.

Opentherm is designed so the pair of wires connecting the Room Unit to the boiler can carry the voltage to power the room unit and the digital data stream between the Room Unit and the boiler.

sorry, I had the remeha isense in my mind when stated about the batteries

the intergas can run the isense without batteries but will not have backlighting (so will not be able to see it in the dark) and if the boiler is switch off you have to set the time, day, month and year again when it the boiler gets switch back on.

How long do the batteries last on the wireless room thermostat?
Usually til the coldest night of the year and when your ill, so you have to walk through an ice box to over eide it on the receiver, which customers never know how 2 do anyway
