Measuring steel pipe to thread

To do this say between two elbows do you measure between were the thread ends internally on both elbows?

Not quite how it works assuming standard taper threads you would always expect one or two threads left on male thread when tight.
If it goes right to end of fitting you have not got threader set up properly.
Only exception would be if for some strange reason you were making a long thread.
Always best to do a trial thread and see how tight it is on fitting.

From the face of the fitting add below for each elbow,
1/2" elbow + 13mm
3/4" elbow + 15mm
1" elbow + 17mm
1 1/4" elbow + 19mm
1 1/2" elbow + 19mm
2" elbow + 24mm

So it wouldn't go all the way in to the finale thread internally on the fitting?

Is this what you have realised has work? So I could measure face to face on two elbows for a piece of pipe and if 1/2inch add 26mm.
 
Yes, it's industry standard for malleable iron fittings, are you using an electric pipe threader?.
