The green/white form is a variant of the NICEIC form which is sold to anyone and is for use by people who are not members of the NICEIC domestic installer scheme or they are carrying out work outside their enrolment scope.
Anyone can buy and use them, wether they are a member of a competent person scheme or not. If he was a memeber of the NICEIC domestic installer scheme I'd have expected him to use a blue form.
