I got sacked because I would not remove the screws on a panel lid in a power station. We were to fit a PLC and needed to know where control wires were but there were very few plans and the panels were missing the ID information.
I say if no tool is required to access the panel it should not have any exposed live parts so in theory OK for us to inspect.
But with a row of bolts it could be back of 11kv panel we had no way of knowing the voltage. If not a power station and the only supply was 400vac I would have looked but not when there was 11kv on site.
So yes good point was made. However if you go into a shop and ask to see the equipment register they will look at you as if your from a different planet! And sorry to say most workers in small firms have no idea as to there responsibility.
Our general education has become too specialised and school, collage, and University leavers can all work computers but have no idea how to even safely empty the ashes from a normal fire or stove. Or have any idea of the dangers of cuts and grazes where any horse is around.
But very few "accidents" are due to one fault. Take the first one.
1) Area congested.
2) Grease on floor.
3) Should not need to move pan while full.
4) No equipment to safely move it.
But English "Accident" means not intended to happen. Although all were avoidable they were still accidents.
I do believe someone tried to take the church to court in USA as the damage was attributed to an act of god!
