In a mil setting it's almost impossible to get it wrong due to the toggle and clasp (ie thing) setup on the hoist side.

In the navy I have known of an incident where the Jack and Ensign was raised together but on the wrong end of the ship. "Reasons in Writing" were the order of the day to the officer in charge.
