judging by the splatter pattern of melted metal, I'd say that there was a threaded stud welded on there with a stud welder and it's been broken off taking the bit it was welded to with it..

or a spot welder..

Looks like a stainless steel bolt was welded on as an earth point, blew apart due to short circuit. Can't think what it is though. Do I get the prize
