Would this fail PD?

There's no way it's a two storey extension, people are letting nit-picking get in the way of reality.

Submit this to the council for comment, if they claim it's anything other than single storey I'll bare my arse in Tesco.

In my opinion, would fail under PD. As the enlargements are connected, the whole addition is taken as a single enlargement, and the enlargement doesn't accord to the provisions of the GPDO.
