What has the vet recommended?
If you really must provide heating especially for the cat then try an electric heater with thermostatic timed controls, set very low but enough to provide just enough heating without making it too warm or costing too much.
And dont belittle a cardboard box, there are plenty of people that sleep in them in sub zero temperatures because they keep the heat in, line it with polystyrene or Kingspan (on the outside) to increase the efficiency of it.
Old cats live outside quite well enough, hey are just pampered when we let them indoors.
