I have actually fitted two new tyres to my own car a few years back.

Purchased online and spent an enjoyable Saturday afternoon fitting them.

The annoying thing about tyres is, even if new, you get a puncture close to the shoulder or wall, it has to be scrapped.

If there is a puncture outside the tread then it needs a vulcanised repair.
Most places don't have the equipment or want to send the tyre away to get it done.

My daughter had to replace a punctured tyre last week due to its side wall being worn down when driven with low pressure.
A week after its replacement, the dashboard monitor flagged up low pressure for the same tyre.
The valve had failed as the centre spindle was poking out. Fortunately I managed to put enough air into the tyre to return car to garage for a fix.
Having maintained my own cars over the last 40+ years, it was my first experience of a valve problem and was shocked to discover that a simple tool exists to screw in the valve spindle.
You learn something every day and my toolbox now has an addition (probably never to be used in anger again)
I'm just thankful I didn't have to struggle with the spare wheel.

Very unusual not to have the valve replaced at the same time as the tyre - it’s standard practice along with balancing.

Maybe it was a new valve - but the core hadn't been screwed in fully?

I doubt tpms valves are changed as a matter of routine.

They aren't 10 a penny like the old valves were
