Then there's the ones who **** off to France but spend half their life on UK internet forums running down the uk, UK haters who only pop back now and again for a spot of NHS treatment in the country they hate.

I suspect that you are aware that emigrants can live in another country for part of the year, continue paying their taxes in UK, which gives them every right to seek medical treatment from the NHS.
Of course, if they are resident elsewhere, and pay their taxes elsewhere (like a few very rich people do), then they're not entitled to NHS treatment, and they'd need a E111, or whatever it is called issued by their adopted country.
I assume the NHS will bill them, on the basis of private treatment, for any treatment received
