What dissuades people from wrongdoing is not the severity of punishment, but the certainty of being caught.
Yes, and timescales. Saying to someone that if they are naughty they might prosecuted 14 years after an incident is not much of a deterrent.

The Inquiry concluded that Studio E bore primary responsibility for the design of the external wall, a crucial element in the disaster. The firm’s role also extended to coordinating other consultants, sub-contractors, and specialists. Despite these extensive responsibilities, the report found that Studio E fundamentally misunderstood the scope of its obligations.

Behind all of that is the actual panel / insulation makers. Obfuscation may figure. The full report would need reading.
 
Successful prosecutions seem unlikely, but the government can and should punish the culpable insualtion panel companies by financial samctions such as keeping them off the tendering list, or introducing civil penalties. A company hit by meaningful fines is sadly likely to react more positively than one of its middle men being thrown in prison.
