There is no legislation which says that the purchaser must fill out any specific form. There is legislation which says that the dealer must provide notification to TVL of the purchaser's name and address. So it's up to the dealer how he obtains that information, and clearly some simply ask for a name and address while others have a policy of having the purchaser fill out a form with the required information. The store asking for the purchaser to make a declaration that a license is in force must be doing that as a matter of company policy for some reason, since it's not a statutory requirement.
