And then they sign and date a HSA1 form and at the end of it all, the abortion is approved.

Maybe the way to resolve this is to come up with a definition of what the two doctors are actually doing in practice. And then debate whether that means approval. Would MBK agree with the above definition? Essentially, narrow the issue down like at a defamation trial.
Yes

If the 2 doctors don't agree with the opinion needed they can't sign it, so it's not APPROVED and can't go ahead.
 
It would be very difficult and in most cases illegal to go ahead without her permission.
I meant the woman can't proceed without the doctors' sign off, in response to "the decision to proceed is the woman's".
