I do believe you're getting it.
Gender reassignmet surgery allows someone to have the genital characterisitcs of the 'other' sex.
It's also possible that those with ambiguous sex may have the biology of the 'other' sex.
Sex is not binary nor immutable.
Current science and medicine cannot make all the necessary procedures for a male to become a female, yet.

I doubt you are really so stupid as to think that surgery changes sex.
If you have your neck lengthened, and your skin tattooed in yellow and black splodges, you become a man with tattooed skin and a long neck. Not a giraffe.
When you applied to have your pay cut by a third, you did not become a woman.
